Upon admission to Mercer, students must complete a Health Information Form.

You may click on this link to download a copy of the  Health Information Form.

This form must be signed by a healthcare provider.

    ALL STUDENTS ARE REQUIRED TO HAVE THE FOLLOWING:

A PPD (tuberculosis screening) valid within the last year.

            Proof of two Measles, Mumps and Rubella shots 

                                                                        OR

                                                                  MMR Titer

           (A copy of the actual lab results must be included)

  A valid health insurance card prior to attending class.**

**Reminder Mercer University require all students who are attending the Atlanta Campus to have health insurance. 

Please note that Pearce and Pearce will verify coverage.

Coverage will be verified each semester.
