Each student must complete a Health Information Form that asks for information about your health, immunizations and health care provider. Your health care provider must sign the Health Information Form.

All students attending the Atlanta Campus must have:
• a PPD (tuberculosis screening) valid within the last year
• proof of two Measles, Mumps and Rubella shots or have blood work showing proof of immunity to Measles, Mumps and Rubella (an MMR titer). A copy of the actual lab results must be turned in with the Health Information Form.
• a valid health insurance policy prior to attending class. A copy of the front and back of the card must be submitted with the Health Information Form.
